How It Works
1. Join Our Network
Complete our provider application. We'll visit your facility, verify licenses, and understand your specific capabilities.
2. Receive Matches
When we have a patient who matches your capabilities, we send you a complete profile including assessment and care needs.
3. Review & Decide
Review the patient information. If interested, we coordinate a tour (virtual or in-person) with the family or discharge planner.
4. Placement
If it's a good fit, we handle paperwork coordination and help facilitate a smooth move-in process.
Our Specializations
We focus on high-acuity patients that general placement services can't handle. If your home has these capabilities, we're looking for you:
High-Acuity Capabilities
- RN-Operated Homes — Registered Nurses on staff or as operators
- Private Duty Nursing (PDN) Contracts — 24/7 skilled nursing coverage
- Ventilator Care — Mechanical ventilation management
- Tracheostomy Care — Trach tube management and suctioning
- Bariatric Care — Specialized equipment and transfer assistance
- Dialysis Coordination — In-center or home dialysis support
- Memory Care — Secured environments for dementia patients
Our Vetting Process
We maintain quality standards for our network to ensure families and discharge planners can trust our recommendations:
Site Visit
We personally visit every home in our network to assess the environment, meet staff, and verify capabilities.
License Verification
We confirm all required licenses are current and in good standing with DSHS.
Reference Checks
We speak with current/former residents' families and healthcare partners when possible.
Ongoing Monitoring
We follow up after placements and maintain regular contact to ensure quality standards.
Fee Structure
Our service is free for families. Adult family homes pay a placement fee only when we successfully match a patient to your facility.
Standard Placement Fee
- 100% of first month's rent — Standard industry rate
- No upfront costs — You only pay when a placement is successful
- No monthly fees — We don't charge for being in our network
- Higher-acuity = higher fees — Ventilator, PDN, and other complex cases command higher monthly rates
Example: A ventilator patient paying $18,000/month = $18,000 placement fee. A basic care patient paying $6,000/month = $6,000 placement fee.
Frequently Asked Questions for Adult Family Home Providers
What are the requirements to join your network?
We require: 1) Valid Washington State adult family home license, 2) Current liability insurance, 3) At least 6 months of operation with positive DSHS survey history, 4) Willingness to accept high-acuity patients, 5) Commitment to our quality standards. We verify all licenses through the Washington State DSHS licensing database and conduct in-person facility assessments.
How do you match patients to our home?
We use a detailed matching process that considers: medical needs, equipment requirements, staffing capabilities, facility layout, location preferences, and payment sources. We never place a patient in a home that isn't fully equipped and staffed for their specific needs. Our pre-screening includes medical record review, family interviews, and care plan development before placement.
What support do you provide after placement?
We provide ongoing support including: 1) 30-day check-ins to ensure smooth transition, 2) Assistance with care plan adjustments, 3) Mediation if issues arise, 4) Connection to additional resources like Medicare-covered therapies or specialized equipment vendors, 5) Regular communication with families to address concerns before they become problems.
How quickly can you fill empty beds?
For standard care beds, we typically fill within 2-4 weeks. For high-acuity beds (ventilator, PDN, etc.), placement can be immediate to 2 weeks depending on current patient needs. We maintain a waiting list of pre-qualified patients and work closely with hospital discharge planners who contact us daily with urgent placement needs.
Do you work with Medicaid/Medicare patients?
Yes, we work with all payment sources including private pay, Medicaid, Medicare (with Private Duty Nursing), long-term care insurance, and veterans benefits. We help families navigate payment options and connect homes with Medicaid providers and PDN agencies for Medicare-covered skilled nursing services.
What happens if a placement doesn't work out?
We have a 30-day trial period for all placements. If the placement isn't working for any reason, we work with both parties to find a solution. If relocation is necessary, we handle the transition at no additional cost to you and work to find a more appropriate placement for the resident. Our goal is successful long-term placements.
